Exploring the Evolution of IOL Technology with Sumitra Khandelwal, MD

As cataract surgery continues to evolve, advancements in intraocular lens (IOL) technology and preoperative diagnostics are reshaping patient outcomes. Balancing cutting-edge technology with established methods is key in IOL selection.

In an interview with HCPLive, Sumitra Khandelwal, MD, a professor of ophthalmology at the Cullen Eye Insitute at Baylor College of Medicine, and medical director for the Lions Eye Bank of Texas, explained how each lens category continues to evolve, with newer options offering distinct advantages and trade-offs.

Khandelwal recently presented a session titled “Pearls for Picking the Perfect IOL” at the Texas Ophthalmological Association (TOA) Annual Meeting in Houston, Texas.

Khandelwal noted that while mono-focal lenses remain the most common choice, advancements in trifocal and astigmatism-correcting IOLs allow for greater spectacle independence—though some patients may experience glare and halos.

She indicated one of the most innovative options available is the light-adjustable lens, which allows post-surgical customization over multiple visits. While offering precise visual outcomes, Khandelwal emphasized the need for patient commitment due to the time-intensive nature of adjustments. This option could be particularly beneficial for those uncertain about monovision, as it allows for refinements after surgery.

Khandelwal also highlighted the evolving role of ocular surface disease management in optimizing cataract surgery outcomes. With an increasing number of pharmaceutical treatments—ranging from anti-inflammatory drops to tear stimulators—the field is experiencing rapid expansion. She noted that understanding how to integrate these treatments effectively will be a key focus in ophthalmology throughout the year.
