A low-cost predictive machine learning model successfully predicted achievement of remission based on Clinical Disease Activity (CDAI) measures among a cohort of patients with 

 after 6 months of treatment with tumor necrosis factor (TNF) inhibitors, according to research published in 

As investigators only used CDAI measures at baseline at month 6, they believe this method demonstrates the achievability of creating regional cohorts to create low-cost models tailored to specific institutions or geographical regions.

Currently, treatment guidelines suggest initiating therapy with methotrexate, followed by either biologic disease-modifying antirheumatic drugs (bDMARDs) or Janus kinase (JAK) inhibitors if methotrexate is ineffective. However, recent studies have favored bDMARDs over JAKs and, therefore, treatment with a TNF is becoming an increasingly popular option for this patient population. Despite these encouraging findings, only 70% of patients initiating a TNF show a favorable response.

“Numerous efforts have been reported to predict the efficacy of TNF in advance,” wrote a team of investigators led by Koshiro Sonomoto, PhD, associated with the University of Occupational and Environmental Health in Japan. “However, access to these advanced technologies may be limited to certain countries and advanced facilities due to cost, labor requirements, and the need for process standardization. Conversely, predictive models based on routine clinical data are more accessible.”

Patients with RA beginning treatment with a TNF as the first targeted synthetic (ts)/bDMARD after inadequate response to methotrexate were included in the analysis. Data were collected from the FIRST registry between August 2003 and October 2022. The analysis of baseline characteristics and 6-month CDAI used a variety of machine learning approaches, such as lasso logistic regression (Lasso), logistic regression with stepwise variable selection, support vector machine, and decision tree, along with 48 factors available in routine clinical practice for the prediction model.

A total of 4706 patients in the FIRST registry initiated treatment with b/tsDMARDs during the study period, of which 2223 were receiving methotrexate. Of these patients, 1630 received a TNF and 79 received a JAK. The average age of patients was 59.2 years, they had an average body mass index of 22.1, a mean disease duration of 75.7 months, and all were Asian. The mean dose of methotrexate was 11.3 mg/week and the mean CDAI score was 26.1.

The models exclusively relied on patient-reported outcomes and quantitative parameters, as opposed to subjective physician input.

Of the approaches, Lasso demonstrated advantages in predicting CDAI remission, with a specificity of 69.9%, sensitivity of 61.7%, and a mean area under the curve of .704. Patients who were predicted to respond to TNF achieved CDAI remission at an average rate of 53.2%, compared with only 26.4% of predicted non-responders.

These results could also help to identify an alternative b/tsDMARDs class for patients who were predicted to be TNF non-responders.

Investigators mentioned limitations, including the decreasing accuracy of the Lasso-generated remission predictive model among a Calendar cohort. In this group, which was split into a 9:1 ratio with a cutoff of October 2019, an increase in censoring was reported in the validation cohort. They suggest this could be due to COVID-19 complications.

“This approach holds the potential to improve rheumatoid arthritis management by reducing the need for trial-and-error approaches and facilitating more personalized and effective treatment strategies,” investigators concluded. “While further validation is necessary, the study also suggests that creating cost-effective models tailored to specific regions or institutions is possible.”

A new systematic review and meta-analysis demonstrated a statistically significant risk of 

among patients with both type 1 (T1D) and 

, compared with non-diabetic individuals.

These diabetic patients demonstrated a 90% greater risk of developing the condition, with sex representing a modifying variable— studies with a higher proportion of men exhibited a lower likelihood of CTS occurrence than those with a higher proportion of women.

“The results of this study can be highly beneficial for health professionals in making well-informed decisions and adopting evidence-based preventive measures for patients with diabetes,” wrote the investigative team, led by Abdollah Mohammadian-Hafshejani, assistant professor of epidemiology, Shahrekord University of Medical Sciences.

The leading cause of sick leave and work disability among working-age adults, CTS demonstrates a serious impact on the global adult population.

 Various health conditions have been linked with the syndrome, including metabolic disorders and obesity, with diabetes having been highlighted as a potential risk factor.

Multiple recent studies have assessed the relationship between diabetes and CTS, with numerous reporting an increased risk of developing CTS in the presence of diabetes.

 On the other hand, some studies have demonstrated no association between diabetes and the occurrence of CTS.

Given the inconsistencies in risk between diabetes and CTS, Mohammadian-Hafshejani and colleagues conducted a systematic review and meta-analysis to investigate the association using all available literature.

 A comprehensive search was conducted in the PubMed, ISI Web of Knowledge, Cochrane, Embase, Google Scholar, and Scopus databases, using diabetes as an exposure and CTS as an outcome, until the end of 2022.

The primary study objective was to assess this association and to determine the relationship’s effect size, with a 95% confidence interval (CI) presented in the articles matching inclusion criteria or calculated based on the provided information.

A total of 2060 articles were initially identified in the electronic database search and 1448 were excluded due to being unrelated to the research topic. These exclusions resulted in 49 potentially relevant papers and further exclusions left a total of 42 observational articles for the study. These articles combined for a study population of 3,377,816 participants between 1991 and 2022.

Overall, results from the meta-analysis revealed the odds ratio (OR) of CTS among individuals with a history of diabetes, compared with those who did not, was 1.90 (95% CI, 1.64 - 2.21; 

 <.001). These data suggest the presence of diabetes statistically significantly increased the odds of CTS by 90%.

A meta-regression was performed to investigate the cause of heterogeneity in study results, with variables including year, study design, sample size, quality of study, and sex ratio (male to female) of participants. According to these data, the sex ratio was the only variable significantly contributing to the heterogeneity in the results of the analyzed studies.

Mohammadian-Hafshejani and colleagues noted the limitations of observational studies stem from the presence of confounding variables when examining the connection between exposure and outcome, despite attempts to control for them. As a result, they urged caution in interpreting these results, without enough evidence to establish a causal link between diabetes and CTS.

“Consequently, it is not possible to infer causal inference relationships between exposure and outcome in these studies, and should only interpret the relationship between variables,” investigators wrote.

Presence of Diabetes Linked to Carpal Tunnel Syndrome Occurrence

Continuous care with the help of a mobile health (mHealth) app was shown to improve knowledge levels and treatment compliance among a cohort of patients with 

“Although the treat-to-target approach has been widely recognized, gout control remains suboptimal,” wrote a group of international investigators led by Ying Wang, MSN, associated with the Department of Rheumatology and Immunology, West China Hospital, West China School of Nursing, Sichuan University, in China. “Therefore, Chinese patients with gout urgently need to improve disease-related knowledge and treatment compliance.”

Continuous care uses specific approaches to ensure patients receive the same level of collaborative, comprehensive care throughout their treatment and management journey. Ideally, this type of care should be highly accessible, convenient, and economical. Currently, these strategies mainly consist of a combination of telephone follow-ups, face-to-face counseling, sending health-related content through social media, and delivering health information to patients via manuals. While these methods have their advantages, they have been shown to exhibit poor accessibility, small coverage, and low efficiency. Other models of continuous care, such as internet-based mHealth apps, may offer improved health services for this patient population.

The single-center, single-blind, parallel-group, randomized controlled trial recruited patients with gout from the West China Hospital of Sichuan University in Chengdu, China, to assess the impact of mHealth-based continuous care on improving treatment compliance and gout knowledge as well as reducing serum uric acid (SUA) levels. Patients were randomly assigned to either the intervention cohort or control cohort.

Participants in the intervention group were given continuous care using an mHealth app for 24 weeks. The app included modules for health records, education materials, and interactive support. Controls received face-to-face health education, paper-based education materials, and patient-initiated telephone consultations.

Both groups participated in a follow-up appointment at 6 months. The individual’s knowledge of gout and treatment compliance were measured at baseline, 12 weeks, and 24 weeks, while SUA levels were measured at baseline and 24 weeks. Efficacy was determined using a generalized estimating equation model and the intention-to-treat principle.

Ultimately, 120 patients were randomized into the intervention (n = 60) and control (n = 60) groups. Most (77.5%, n = 93) patients completed the entire study. Baseline measurements, clinical characteristics, and sociodemographic variables were comparable between groups (

However, patients placed in the intervention group reported significant improvements in gout knowledge levels over time compared with controls (β = .617, 95% confidence interval [CI] .104 — 1.129; 

= .02 and β = 1.300, 95% CI .669 — 1.931; 

Although there was no significant difference in treatment adherence at the 12-week mark among groups (β = 1.667, 95% CI −3.283 — 6.617; 

= .51), a statistical difference was noted at week 24 (β = 6.287, 95% CI 1.357 — 11.216; 

= .01). No significant differences in SUA changes were observed between groups, although levels were below baseline at week 24 in both cohorts (

Investigators noted limitations including only recruiting patients from a single hospital in Sichuan, China, with a disproportionate number of male patients, which could limit the generalizability of the findings. Eligible patients also needed to be able to use smartphones and have certain learning abilities, which could further hinder generalizability among all patients with gout. Additionally, sustainability could not be established as the study was 24 weeks long. Investigators encourage future multicenter studies—which would include patients with other rheumatic diseases as well—with a longer follow-up duration to confirm these results.

“We suggest incorporating this intervention modality into standard continuous care for patients with gout,” investigators concluded.

Discussing Risankizumb, Differences in IL-23 Inhibitor Epitopes with Christopher Bunick, MD, PhD

The importance of interleukin (IL)-23 to the pathogenesis of plaque 

 and psoriatic disease in general has become increasingly apparent in the past few years within the sphere of dermatology research.

This area of research was explored recently in a study titled ‘Structural Basis for p19 Targeting by Anti–IL-23 Biologics: Correlations with Short- and Long-Term Efficacy in Psoriasis,’ with one of the study’s authors being Yale School of Medicine associate professor Christopher Bunick, MD, PhD. Bunick spoke to HCPLive about his team’s findings and their implications.

“What we've realized is that as there's been an evolution in generations of IL-23-targeting biologics, that's a progression from targeting the p40 subunit, ustekinumab to the p19-specific subunits of IL-23, which are risankizumab, tildrakizumab, and guselkumab,” Bunick explained. “So this evolution, how we target interleukin-23 has really become clinically-apparent as these IL-23-targeting biologics have come to really dominate the clinical realm of psoriasis treatment.”

Bunick noted that his team sought to better understand the differences in these IL-23 biologic inhibitors and their relationship to psoriasis pathogenesis. He was asked how the structural differences in IL-23–binding epitopes among these inhibitors impact their clinical efficacy, particularly in terms of binding affinity, kinetic values, and long-term outcomes.

The team used epitope data that had been derived from hydrogen-deuterium exchange or crystallographic experiments, correlating the molecular properties of the inhibitor epitopes to their kinetic values, binding affinity, and clinical effectiveness scores for psoriasis.

The investigators had found that there were substantial correlations between the epitopes' surface areas, an inhibitor's dissociation rate constant (k

). They also found that epitope surface area, K

 each had been linked to both shorter-term and longer-term Psoriasis Area Severity Index (PASI)-90 response efficacy.

“What was really fascinating is that, when we looked at the different IL-23 biologics, there was certainly a difference in surface area among the epitopes,” Bunick said. “Risankizumab had the largest followed by guselkumab rather closely and then much lower down you had ustekinumab and then tildrakizumab.”

Bunick was later asked about some of the specific factors which could contribute to this superior efficacy with some of these drugs over others, and how they compare to the efficacy of the other IL-23 inhibitors.

“In the clinic, we know that risankizumab has been a very strong performer, if not the best performer, of the p19-specific biologics in treating plaque psoriasis,” Bunick said. “We feel that our paper and our analysis has done this to shed molecular insights into what really makes risankizumab different as a p19-targeted biologic, and I think there's a few key factors.”

Bunicke explained that the first factor is that the drug is binding a key loop region that no other biologic is targeting. The second factor is that it has the largest epitopes surface area, and he noted that this is significant because it has the lowest k

“That means risankizumab is forming the strongest biologic complex with the IL-23,” Bunick said. “And that has clinical impact because if you're soaking up IL-23 with risankizumab and that complex is not coming apart easily, that IL-23 can't signal and can’t perpetuate the signal that's driving psoriasis.”

Shame derived from structural or self-stigma may inhibit patients from obtaining treatment for their 

. This could be because patients might downplay their drinking problems. By internalizing cultural norms surrounding 

, those with alcohol-related liver disease might fear being labeled as an “alcoholic.” 

The healthcare system was designed to separate addiction and hepatology care. Patients would see their alcohol or addiction specialist for alcohol-related issues and would see their hepatology specialist for liver problems. This contributes to patients feeling like they are the ones to be blamed—that their alcohol use was the sole reason they had developed liver disease.

However, this is not true. Although alcohol can lead to liver problems, other causes such as genetics or obesity can lead to liver issues, too.

Research suggests work needs to be done on de-stigmatizing alcohol-related liver disease. A 

 revealed limited usage of non-stigmatization language for AUD and alcohol-related liver disease, underscoring the need for more patient-centered language in transplant centers.

 special report, Hersh Shroff, MD, MPA, moderated a conversation between hepatology specialist Jay Luther, MD, and addiction specialist Chris Kahler, PhD, to discuss the stigmatization of AUD and how hepatologists and addiction specialists can work to collaborate to give patients improved care.

The first part of the report focused on the different types of stigma and how the stigma around AUD may contribute to delays in seeking care for liver disease. The second half of the discussion pivoted to how the collaboration between hepatology and addiction specialists may reduce the stigma surrounding alcohol-associated liver disease.

“To expect that the individual necessarily will have full control over their capacity to say, ‘I can stop drinking tomorrow’ is not realistic for many people,” Kahler said. “It involves not just the involvement of psychiatrists, but social workers, or psychologists, other people who can think about the social determinants of health because those are often driving factors both in alcohol use, but also in delays in seeking care.”

He added the importance of peer support or recovery coaches to share experiences with to reduce the stigma. He also said screening for unhealthy alcohol use is important, rather than a doctor telling their patient, “I think you need to quit drinking.” Kahler stressed the importance of referring the patient to a liver specialist so they can examine actual negative impacts instead of making assumptions.

“Catching patients early or getting patients in who are at high risk for developing disease, so they never develop disease… are critical pieces to the puzzle that…need more attention,” Luther said.
