American Diabetes Association (ADA) Scientific Sessions 2026

, in New Orleans, Louisiana, gathered together some of the greatest minds in endocrinology to discuss the latest major clinical trials and technological advancements in diabetes and 

 care. From Eli Lilly’s triple receptor agonist retatrutide to the ultra-long-acting GLP-1 receptor agonist berobenatide made by Pfizer, this year’s meeting was dominated by major steps forward in incretin-based therapies.

Additionally, key evidence was put forward during the conference to support the broader application of continuous glucose monitors (CGMs) and continuous ketone monitors (CKMs) in monitoring and protecting patients with 

. The CONNECT trial in particular highlighted CGMs’ efficacy in patients with T2D who are not on insulin.

TRANSCEND-T2D-1: Retatrutide Demonstrates Superior Weight Loss and A1c Results Versus Placebo


Eli Lilly’s groundbreaking GLP-1/GIP/glucagon triple agonist retatrutide substantially outperformed placebo in A1c reduction and weight loss among patients with T2D. Presented by Harpreet Singh Bajaj, MD, MPH, this study marks the first phase 3 trial of a triple agonist in this patient population, and allowed 90% of enrolled patients to achieve an HbA1c below the ADA’s T2D target of 7%.


TRIUMPH-1: Retatrutide Substantially Lowers Weight, BMI in Patients With Obesity or Overweight


Presented in tandem with TRANSCEND-T2D-1, this phase 3 study compared retatrutide to placebo in reducing body weight among patients with obesity. Additionally, the trial included 2 basket trials for knee osteoarthritis pain and moderate-to-severe obstructive sleep apnea, respectively. Retatrutide demonstrated a substantial reduction in body mass index (BMI), WOMAC pain subscale scores, and apnea-hypopnea index for patients with overweight, knee osteoarthritis, and obstructive sleep apnea, respectively.


Survodutide Shows Fat Mass Loss, Liver Fat Reductions in Patients With Obesity Without T2D

In the phase 3 SYNCHRONIZE-1 study, Boehringer Ingelheim’s investigative glucagon/GLP-1 receptor dual agonist survodutide demonstrated substantial improvements to metabolic health and weight loss in patients with overweight or obesity without T2D. CONNECT Trial Backs CGM for Non-Insulin Type 2 Diabetes Management


The CONNECT randomized controlled trial has demonstrated the importance of CGMs in managing T2D in adults who are not on insulin. The trial demonstrated the devices’ efficacy in reducing hyperglycemia, thereby reducing A1c, compared to a routine care control group. CONNECT also analyzed patient-reported satisfaction and distress, indicating reduced greater satisfaction and reduced distress among patients in the CGM group.

Key Off-Label Treatment Options in Dermatology, With Matthew Zirwas, MD

 Annual Summer Dermatology Conference, dermatologist Matthew Zirwas highlighted several practical off-label treatment strategies during the session 

‘No Label No Trouble: A Panel Review of Off-Label Medication Use,’ 

emphasizing the continued importance of affordable and versatile therapies in dermatology.

Oral roflumilast, a phosphodiesterase-4 (PDE4) inhibitor, is an underappreciated off-label option with efficacy approaching that of some biologic therapies.

Linezolid, an antibiotic more commonly linked with treatment of resistant bacterial infections, can be effective for difficult-to-treat cases of hidradenitis suppurativa and chronic folliculitis.

Generic tofacitinib is a development worth watching in the coming years in the dermatology field.

 following the presentation, Zirwas noted, despite the growing number of targeted therapies entering the field, clinicians will continue to face notable treatment gaps for many dermatologic conditions that lack US Food and Drug Administration (FDA)-approved options.

“I think the most important thing I'd like to get across to people is a drug called oral roflumilast,” Zirwas said. “So, this is a PDE4 inhibitor, just like apremilast, that we use for psoriasis, but it is substantially more effective.”

Off-Label Roflumilast for Psoriasis and Other Conditions:

Zirwas pointed to the presence of oral roflumilast as 1 of the most important takeaways from the session, describing the phosphodiesterase-4 (PDE4) inhibitor as an underappreciated option with efficacy approaching that of some biologic therapies while maintaining a favorable safety profile similar to that of apremilast. According to Zirwas, oral roflumilast offers a unique combination of effectiveness, accessibility, and affordability, with a cash price of only a few dollars per month.

Although psoriasis is 1 potential use case, he suggested the medication may be even more valuable in a variety of inflammatory dermatologic diseases that have limited treatment options, including granuloma annulare, cutaneous lupus, cutaneous dermatomyositis, and palmoplantar pustulosis. He also highlighted its utility as an adjunctive therapy for patients already receiving biologics, noting that it can provide an additional clinical benefit without significantly increasing treatment-related risk.

Off-Label Linezolid for HS and Chronic Folliculitis:

Zirwas also discussed the off-label use of linezolid (Zyvox), an antibiotic more commonly associated with treatment of resistant bacterial infections. He noted that short courses of linezolid can be highly effective for difficult-to-treat cases of hidradenitis suppurativa (HS) and chronic folliculitis, particularly in patients who continue to experience disease activity despite other therapies. Limiting treatment to approximately 10 days, he explained, helps preserve the medication’s favorable safety profile while providing meaningful clinical improvement.

Looking ahead, Zirwas predicted that off-label prescribing will remain an important component of dermatologic practice, largely because many rare or less common skin diseases are unlikely to receive dedicated FDA-approved therapies. He also identified generic tofacitinib as a development worth watching in the coming years, suggesting that lower-cost JAK inhibition could eventually expand treatment options across a broad range of dermatologic conditions.

In this final episode, Dr. Cusano asks Dr. Dolores Shoback how she tailors communication for patients who are not meeting treatment goals, experiencing complications, or struggling with the burden of long-term disease management.

Communication, Multidisciplinary Collaboration, and Future Outlook in Hypoparathyroidism

Dr. Shoback emphasizes the importance of involving family members early, since patients are often trying to manage the disease alone. She highlights community support groups—including the Hypoparathyroidism Association, which holds meetings for patients and caregivers—as a valuable resource for both information and peer support. Dr. Cusano notes that two of her own patients found each other through a support group and have benefited greatly. When significant depression, anxiety, or other mental health complications are present, referral to a mental health provider—whether for medication, counseling, or family therapy—is essential. She underscores that there is no single path for these patients, and that intensive, individualized care with frequent appointments and maximum support is often necessary.

On adherence, Dr. Shoback advocates for engaging patients in actively tracking their own labs and medications, using computers or phones as tools. Positive feedback, clear rescue dosing instructions, and empowering patients to feel "in the driver's seat" of their own management are central strategies. She frames this as applying the same principles used in other chronic disease management—tailored to the individual.

Dr. Lubitz adds that consistent messaging with primary care providers is critical, particularly to prevent misinterpretation of intentionally low-normal calcium values as a laboratory emergency requiring dose escalation.

Dr. Cusano then opens the floor to all panelists on the future of hypoparathyroidism management. Dr. Khosravi anticipates refinement of guidelines as real-world and randomized data mature, better surgical identification techniques such as autofluorescence, potential cell-based therapies, point-of-care calcium monitoring, and precision medicine approaches like encaleret for ADH1. Dr. Ferenczi urges greater referral of patients with genetic forms who remain undiagnosed or undertreated, and emphasizes that emerging therapies can meaningfully improve quality of life for those patients if they reach appropriate specialists.

DBV Technologies Screens First Infant in THRIVE Peanut Patch Trial

DBV Technologies announced the first participant has been screened in THRIVE, a phase 2 study evaluating the Viaskin Peanut patch in infants aged 6 through 12 months with 

, with principal investigator Douglas Mack, MD, assistant clinical professor in the department of pediatrics at McMaster University, screening the first participant.

The single-arm, open-label study will assess the efficacy and safety of daily epicutaneous immunotherapy (EPIT) via the Viaskin Peanut patch in this youngest-ever population enrolled in a peanut allergy trial of this scale. All participants will wear the patch daily for 36 months while maintaining a peanut-free diet, followed by a peanut food challenge to determine the consumption regimen for months 37 through 48.

The primary endpoint is ad libitum peanut consumption, defined by prespecified criteria including tolerated single-sitting intake, longitudinal consumption patterns, and caregiver-reported outcomes. Mack said choosing ad lib consumption as the bar reflects an ambition beyond desensitization alone.

"Ad lib, to me, is the next stage in this," Mack said. "If we can induce not just a desensitization but help these kids function not dissimilar to children that were never allergic in the first place, to me that's a marker of true long-term change, remission, tolerance, whatever you want to call this.”

Mack also addressed a practical question around the preverbal status of infants at enrollment. Because the ad lib consumption assessment occurs in the fourth year of the study, most participants will be fully verbal by that point, making caregiver and patient-reported outcomes feasible.

THRIVE builds on results from earlier DBV trials in toddlers aged 1 through 3 years (EPITOPE) and children aged 4 through 7 years (VITESSE), which demonstrated desensitization benefits.

Mack noted the 3-year data from older cohorts already supports the biological plausibility of treating this age group but said the window being targeted here is distinct.

"If we are going to induce long-term change for these patients, we need to start early and capitalize on this early window," he said. "THRIVE will collect valuable data by assessing these patients between 6 and 12 months of age, earlier than we've ever done it. It really makes it the earliest peanut-allergic group we've ever assessed in a clinical trial of this size using EPIT."

On practical considerations, Mack said the counseling burden for families is real but manageable. He drew a parallel to LEAP-era conversations around peanut exposure in early life, noting that most peanut-allergic infants are already living peanut-free. He also pointed out that patch adherence concerns familiar from older cohorts may be less pronounced in this age group, since infants lack the coordination to remove the patch themselves and have lower activity levels in the first year.

Safety monitoring will focus on local site reactions, the most consistently observed adverse event across Viaskin trials. Mack said he does not anticipate anaphylaxis given the low allergen dose, roughly 1/1000 of a peanut, and expects the tolerability profile to mirror what has been seen in prior studies.

Looking ahead, Mack described a potential shift in clinical practice if THRIVE data are positive: moving toward immediate treatment at diagnosis rather than watchful waiting.

“I think what we’re going to see is a movement toward very early treatment, almost like a salvage, so the patient has a reaction, they get tested, and we move straight to an active treatment,” he said. "For many families, [oral immunotherapy] doesn't work [due to] picky eaters, preexisting eosinophilic esophagitis, [and] uncontrolled asthma. We offer both oral immunotherapy and clinical trial patch immunotherapy, and we have a lot of families who are choosing the patch.”

In this episode, Dr. Cusano asks Dr. Andrea Ferenczi to walk through preoperative risk identification, counseling, and postoperative management for patients undergoing thyroid or parathyroid surgery at risk for developing hypoparathyroidism.

Surgical Considerations and Perioperative Management of Hypoparathyroidism

Dr. Ferenczi explains that American Thyroid Association statements and the surgical literature have converged on a consistent set of risk factors: total thyroidectomy, autoimmune thyroid disease, central neck dissection, substernal or retrosternal goiters, surgeon inexperience, and malabsorptive conditions. Additional independent risk factors include preoperative hypocalcemia or vitamin D deficiency, inability to visualize or preserve parathyroids intraoperatively, reoperation, large goiters, female sex, Graves disease, and Hashimoto's thyroiditis. She recommends obtaining baseline serum calcium, PTH, and 25-hydroxyvitamin D before any planned bilateral operation. Correcting vitamin D deficiency preoperatively is a modifiable risk factor that should always be addressed. If baseline calcium is already low-normal or below normal, supplementation can be started preoperatively.

She stresses the importance of distinguishing transient hypoparathyroidism—common and usually resolving within weeks to months—from permanent disease, defined as persisting beyond 12 months. Patients should be counseled candidly that permanent disease means lifelong medication and monitoring. In high-risk patients, surgeon volume matters, and parathyroid autotransplantation should be considered, as it is associated with more transient but less permanent hypoparathyroidism.

Postoperatively, early PTH measurement is the key triage tool: a PTH above 10 pg/mL at 12 to 24 hours post-thyroidectomy makes permanent hypoparathyroidism very unlikely, while a PTH below 6 pg/mL at four to six hours is a strong predictor of permanent disease. She recommends checking serum calcium within 24 hours, teaching patients to recognize symptoms of hypocalcemia—perioral and fingertip tingling, muscle cramps—before discharge, and providing explicit rescue dosing instructions. Most patients should be reassured that early hypocalcemia is often transient, and a definitive diagnosis of permanent hypoparathyroidism should not be declared until 12 months have elapsed, as late recovery can occur. For those who do progress to permanent disease, the postoperative period is the ideal time to establish the long-term monitoring relationship.
