Despite previous uncertainty, the use of nonsteroidal anti-inflammatory drugs (NSAIDs) was not linked to any increase in cardiovascular event rate in patients with 

 In fact, treatment with NSAIDs decreased these events by 12% among this patient population.

NSAIDs have been shown to be both cardiovascular beneficial as well as cardiovascular hazardous because of the potential for hypertensive, proarrhythmic, and prothrombotic side effects. Gout flares are often treated with anti-inflammatory treatments, including NSAIDs, low-dose colchicine, and/or glucocorticoids.

 Therefore, investigators determined the risk of cardiovascular adverse events associated with NSAID treatment among patients with gout.

“In patients with inflammatory conditions, such as gout, the anti-inflammatory effects of NSAIDs might balance out their cardiovascular hazard,” wrote a team of investigators led by Anne Bech-Drewes, a PhD student associated with the Department of Clinical Medicine at Aarhus University. “This risk–benefit balance is important for clinical decision-making. However, no study has examined NSAID-associated cardiovascular risks in patients with gout.”

A nationwide, population-based, case-crossover study identified all Danish patients aged ≥ 18 years diagnosed with gout between 1997 and 2020 who experienced a cardiovascular event.

Cardiovascular events included congestive heart failure, atrial fibrillation or flutter, myocardial infarction, ischemic stroke, and cardiovascular death. Exposure was the use of NSAIDs overall and were further subcategorized by type (ibuprofen, naproxen, and diclofenac), according to data from the Danish National Prescription Registry. The dates 300, 240, 180, and 120 were used prior to the outcome date as reference dates and the Mantel-Haenszel method calculated the odds ratios (ORs) with 95% confidence intervals (CIs) of the link between cardiovascular events and treatment with NSAIDs.

The cohort included 59,150 patients with a first-time gout diagnosis (12%) or those who filled a prescription for allopurinol (88%). Among the gout cohort, most (68%) were male, the median age was 72 years, 29% had no comorbidities, and 27% had a severe comorbidity burden. The most common comorbidities were hypertension (38%), additional inflammatory diseases (19%), degenerative rheumatic disease (18%), diabetes (16%), chronic pulmonary disease (15%), and chronic kidney disease (9%). Approximately half (52%) were receiving diuretics.

Overall, NSAID use decreased the odds of a cardiovascular event by 12% in patients with gout (OR = .88, 95% CI: .85 – .91). These decreased odds were reported in patients receiving both ibuprofen (OR = .92, 95% CI: .88 – .97) and naproxen (OR = .85, 95% CI: .74 – .97); however, this was not observed in the diclofenac cohort (OR = .97, 95% CI: .90 – 1.05).

Additionally, treatment with NSAIDs was linked to decreased odds of all individual components, including congestive heart failure (OR = .89, 95% CI: .83 – .94), atrial fibrillation/flutter (OR = .80, 95% CI: .75 – .85), myocardial infarction (OR = .91, 95% CI: .84 – .99), ischemic stroke (OR = .88, 95% CI: .81 – .95), and cardiovascular death (OR = .60, 95% CI: .56 – .65) when compared with not using NSAIDs. Any NSAID treatment was linked to a decreased risk of cardiovascular events in patients without a previous cancer diagnosis (OR = .87, 95% CI: .84 – .90).

After stratifying by sex, the data remained robust; however, the interpretation of the age- and comorbidity-stratified results was hindered by low precision.

Investigators noted the large cohort of patients strengthened the study. Additionally, selection bias was reduced as these patients had free access to health care and the long-term follow-up period was virtually complete. However, there is a risk of misclassifying in-hospital NSAID users or over-the-counter ibuprofen users as non-users.

“This information may be taken into consideration when selecting NSAIDs for patients with gout,” investigators concluded.

The proof-of-concept feasibility study showed that patient-led monitoring supported the maintenance of allopurinol adherence and helped patients attain and maintain target urate concentrations, potentially reducing the incidences of gout flares.

“This approach to gout management has the potential to shift clinical practice toward empowering people to be invested in their gout management,” wrote the investigative team, led by Sophie L. Stocker, PhD, school of pharmacy, faculty of medicine and health, University of Sydney.

Despite the effectiveness of ULT, such as allopurinol, for long-term management of gout, up to half of patients discontinue therapy within the first six months in Australia. Factors impacting adherence to ULT include education for its role in preventing gout flares and the experiences of healthcare providers in providing gout management advice.

 Self-monitoring, using a biomarker, is recognized by global health organizations, including the World Health Organization (WHO), for allowing active patient participation in their health care.

Thus, self-monitored point-of-care urate level testing may improve adherence to ULT by supporting patients in understanding how the therapy impacts gout.

 Despite the availability of effective medicine, a measurable biomarker, and POC devices, Stocker and colleagues noted that self-monitoring remains underexplored in gout. In this study, the team examined the impact of urate self-monitoring on adherence to allopurinol, urate concentration control, and the incidence of gout flares.

The study was conducted from June 2021 to April 2023 and recruited patients across Australia from a database of people with gout or by advertisements on social media. Participants were trained on a POC device to self-monitor urate concentration and received allopurinol every 3 months. Each participant manually recorded urate concentrations using the POC device at least once a month for 12 months and these data were collected during monthly telehealth visits with a study investigator.

Adherence to the medication was measured using medication event monitoring technology on the bottle of allopurinol. Participants were monitored for time spent below the target urate concentration (<0.36 mmol/L) and were informed staying below would reduce their risk of gout flares.

Overall, 32 patients with gout were enrolled in the analysis—most were male (94%) and more than 50 years old (74%). The most common dosing regimen of allopurinol was 300 mg one-daily and the average urate concentration at baseline was 0.33 mmol/L. Each participant recorded their urate concentration an average of 18 times and measured consistently throughout the 12 months.

A total of 9 participants remained within the target urate concentration range, while a single participant was above the urate concentration range, for the study period. The time spent within the target urate concentration range was increased 1.3-fold (79% to 100%; 

 = .346) from quarter 1 to quarter 4 of the 12 months.

Meanwhile, approximately half of patients (48%) reported gout flares. Among these participants, the incidence of gout flares decreased by 1.6-fold (8 to 5; 

 = .25) during the study period. Regarding adherence, there were 1315 missed doses of ULT during the study, but 91.2% of doses were taken as prescribed.

Moreover, 773 events of suboptimal implementation were identified, including 58 occasional missed doses, 619 repeated missed doses, 92 repeated drug holidays, and 4 occasional drug holidays. Most (94%) of the study population persisted with allopurinol, with only two discontinuations over 12 months.

Stocker and colleagues noted the persistence in allopurinol may reflect the real-time feedback on urate control provided by self-monitoring, as patients can contextualize behavior and inform their understanding of the inflammatory condition.

“Given that most people with gout discontinue ULT within six months of their first prescription, the evaluation of urate self-monitoring in people with gout who are initiating or reinitiating ULT would be of interest because real-time feedback on urate control may assist in encouraging people with gout to persist with ULT, particularly when the risk of gout flares is high,” they wrote.

 hosts Diana Isaacs, PharmD, an endocrine clinical pharmacist, director of Education and Training in Diabetes Technology, and codirector of Endocrine Disorders in Pregnancy at the Cleveland Clinic, and Natalie Bellini, DNP, program director of Diabetes Technology at University Hospitals Diabetes and Metabolic Care Center, tackle a pair of related topics: what the STEP HFpEF DM trial from ACC.24 means for the diabetes community and the need for fair and equitable allocation of incretin-based therapies in the face of ongoing drug shortages and overwhelming demand.

Presented at ACC.24 and simultaneously published in the 

STEP HFpEF DM was a randomized, double-blind, placebo-controlled trial that randomized 616 participants in a 1:1 ratio to semaglutide 2.4 mg or placebo therapy for 52 weeks. Conducted across 108 sites in 16 countries across Asia, Europe, and North and South America, inclusion criteria for the study required patients to have a left ventricular ejection fraction of 45% or greater, a BMI of 30 kg/m2 or greater, and type 2 diabetes.

Like the original STEP HFpEF trial, STEP HFpEF DM had dual primary endpoints, which were change from baseline in Kansas City Cardiomyopathy Questionnaire clinical summary score (KCCQ-CSS) and change in body weight. STEP HFpEF DM also included multiple secondary confirmatory endpoints, these included change in 6-minute walk distance, a hierarchical composite end point of death, heart failure events, and differences in the change in the KCCQ-CSS and 6-minute walk distance, and the change in the C-reactive protein (CRP) level.

Results of the trial indicated use of semaglutide 2.4 mg was associated with a mean change from baseline KCCQ-CSS of 13.7 points compared to a 6.4-point change with placebo therapy (Estimated treatment difference [ETD], 7.3 points; 95% CI, 4.1 to 10.4; 

< .001). Analysis of body weight suggested the mean change from baseline was −9.8% with semaglutide 2.4 mg and −3.4% with placebo therapy (ETD, −6.4 percentage points; 95% CI, −7.6 to −5.2; 

< .001). Further analysis indicated pointed to benefit withsemaglutide 2.4 mg over placebo therapy for change in 6-minute walk distance (estimated between-group difference, 14.3 meters; 95% CI, 3.7 to 24.9; 

= .008), the hierarchical composite endpoint (win ratio, 1.58; 95% CI, 1.29 to 1.94; 

 < .001), and change in CRP level (estimated treatment ratio, 0.67; 95% CI, 0.55 to 0.80; 

Fair and Equitable Allocation of Incretin-Based Therapies

 an editorial penned by a group of 5 from the Department of Medical Ethics and Health Policy at the Perelman School of Medicine of the University of Pennsylvania and the Bergen Center for Ethics and Priority Setting at the University of Bergen brought forth ethical perspectives on the fair allocation of GLP-1 receptor agonists and dual GLP-1/GIP receptor agonists in the face of overwhelming demand and changing coverage of these agents.

In their editorial, the group proposes a 4-tiered system for allocation based on objectives of care and specified distribution criteria. In hierarchical order, these objectives are: to minimize potential years of life lost, prevent imminent medical complications, prevent future medical complications, and improve quality of life. In their discussion, Isaacs and Bellini discuss their real-world experience in balancing this issue as new data and indications continue to emerge.

A comprehensive systematic review and meta-analysis identified an association between 

 and suicidal tendencies, with age serving as a predictive risk factor.

The investigation, involving more than 30 studies and nearly 6 million individuals, revealed the association between suicidal behavior and visual impairment was particularly pronounced among adolescents reporting vision struggles.

“This finding emphasizes the importance of eye health to overall mental well-being. It is recommended that clinicians remain attentive to the elevated risk and be ready to implement suitable suicide prevention measures when required, especially when dealing with adolescents,” wrote the investigative team, led by Young Kook Kim, MD, PhD, department of ophthalmology, Seoul National University College of Medicine.

A significant public health concern, a growing body of literature has addressed suicide and suicidal ideation, but the underlying causes can be complex and multifactorial. Risk factors for suicidal ideation can differ from a transition to behavior, where an individual may attempt or complete suicide. Family history, mental disorders, chronic illness, and sociodemographic factors are each known as significant risk factors for suicide.

Nationwide survey data from the United States found that approximately 88% of participants considered eye health a “critical component” of their overall well-being—blindness was ranked as the most severe conceivable health outcome.

 Visual impairment can thus implicate a patient’s health outside the scope of clinical ophthalmology.

Systematic examination has revealed a notable level of impaired quality of life, social isolation, decreased independence, and prevalence of depression in patients with visual impairment, matching many risk factors for suicide.

 Owing to variability among available literature, the current systematic review and meta-analysis investigated the association between visual impairment and suicide, with an assessment of risk levels and potential contributing variables.

A search was conducted in key medical databases, including PubMed, Embase, Scopus, and the Cochrane Library, from inception to February 2024. All published studies relating to visual impairment and suicide were considered, without restriction in terms of study design, publication date, or language. Numerous meta-regression analyses were performed to identify potential factors contributing to the link between visual impairment and suicide risk.

After identifying 3239 articles in the systematic search, 101 studies were considered potentially relevant, and a full-text review left 31 studies for analysis. The 31 population-based studies comprised 5,692,769 individuals—more than half (52%) of patients were female and the mean age was 48.4 years.

 = .002). Overall, suicide death showed moderate heterogeneity, while high heterogeneity was identified for suicidal behavior and suicidal ideation.

After performing random-effects meta-regression analyses, Kim and colleagues pointed to participant age as a potential risk factor. Subgroup analysis comparing effect sizes to mean age revealed the pooled OR for studies with adolescent patients was 9.85 (95% CI, 4.39 - 22.10), the highest value among the age groups. The second highest OR was found among individuals aged ≥65 years at 6.66 (95% CI, 2.95 - 15.00).

Kim and colleagues noted the limited number of studies addressing adolescents with visual impairment and suicide risk suggests the importance of centering additional research on this population.

“Since the etiology of suicidal behavior is complex, and because diverse risk factors are associated with different individual and cultural contexts, further research is warranted to determine which factors may modulate the risk of suicide in patients with visual impairment,” they wrote.

Visual Impairment Associated with Heightened Risk of Suicidal Tendencies

 with unexplained arthralgia and non-inflammatory symptoms are at a far greater risk of 

 development, according to new findings, providing new information on subclinical PsA.

These findings resulted from new research conducted due to the lack of real-world data on subclinical PsA, which is described as the existence of arthralgia in psoriasis patients. This was considered invaluable given the noted higher risk of PsA among those with subclinical PsA.

The new analysis was led by Alen Zabotti, from the department of medical and biological sciences at the University Hospital Santa Maria della Misericordia Rheumatology Clinic in Udine, Italy. Zabotti et al. acknowledged that a prior systematic literature review had indicated the lack of data on the transition between psoriasis to PsA.

“These could help to better define outcomes for PsA prevention studies in the context of prevention/interception of PsA and early diagnosis,” Zabotti and colleagues wrote. “Therefore, this study investigated the subclinical stage of PsA in terms of risk of progression and clinical symptoms, and to describe the clinical features of new-onset PsA in (psoriasis) cases.”

The investigators assessed data drawn from 2 ongoing European prospective cohorts of individuals with psoriasis, looking at both the Italian cohort, RAPSODI (Ultrasonographic Risk Factors to Develop Psoriatic Arthritis in Psoriatic Patients with and Without ClinIcal Arthralgia), and the German PACE cohort (Psoriasis and Psoriatic Arthritis Cohort Erlangen), both of which were longitudinal studies.

Subjects featured within the 2 cohorts had been given a diangosis with definite psoriasis of the skin or nails by dermatologists. They were also enriched for PsA risk, with many having arthralgia suggesting a potential risk of PsA development.

The investigators looked at study participants that did not have clinical PsA by the time of their entry to the cohort and those with no visible swelling of joints. Subjects with a history of enthesitis, synovitis, dactylitis, or inflammatory back pain were not included in the research, and those who were included were reassessed every half year, being told by the team to report inflammatory symptoms to their designated rheumatologists.

In the period between January 2017 - December 2022, there had been 215 Italian participants enrolled in RAPSODI with psoriasis, of which 66.0% were shown to have baseline arthralgia indicating the presence of subclinical PsA. Within those in the PACE cohort, 169 participants in total between January 2011 - July 2018, all were determined to have subclinical PsA.

The investigators diagnosed new-onset clinical PsA at the time of follow-up through the use of experienced rheumatologists and with CASPAR criteria fulfillment. The team would also characterize the manifestations of subjects’ new-onset clinical PsA by enthesitis, dactylitis, peripheral arthritis, and axial involvement.

Additionally, the team recorded participants’ clinical features at the time of PsA diagnosis, some of which included enthesitis index, joint counts, and presence of dactylitis.

There were 384 individuals given a diagnosis of psoriasis who took part in the study, as well as an average follow-up duration of 33.0 months. Additionally, 80.9% of these individuals were shown to have subclinical PsA, indicating there had been an incidence rate of PsA of 7.7 per 100 patient-years.

The investigators reported that their subclinical PsA patients had a significantly higher risk of developing PsA as opposed to subjects that had psoriasis exclusively (HR=11.7, 95% CI 1.57 to 86.7, 

 = .016). The research team noted that the cumulative incidence function (CIF) estimated the probability of new-onset PsA was about 9.4% (95% CI 4.7% - 10.6%) by the 12-month mark and that it was 22.7% (95% CI 17.2% - 28.6%) by the 36-month mark.

A notable finding was that 58.9% of subjects had been shown to have inflammatory symptoms during the months immediately preceding their diagnosis of PsA, but 83.9% were found to have non-inflammatory symptoms before their PsA diagnosis. Additionally, the most frequent pattern of presentation was shown to be peripheral joint swelling for 82.1% of those with PsA.

“We have confirmed that patients with (psoriasis) experiencing unexplained arthralgia, also reporting non-inflammatory symptoms, are at a significantly elevated risk of developing PsA,” they wrote. “Additionally, peripheral arthritis, mainly oligoarthritis, is the most common PsA presentation in cohorts with PsO enriched for arthralgia.”
